Thank you for taking the time to fill out this evaluation. I ask for this feedback as part of the discernment process for renewing the call of your pastor, but the questions are designed to evaluate the overall ministry of your church. Your answers to this survey are confidential and your name will not be associated with your comments. However, the answers you supply will be compiled along with all the responses in a single format that will be discussed together with your pastor at a board meeting.

The entire survey is designed to take you about 20-30 minutes to complete. Long answers are not necessary but be as thorough as possible and feel free to write as much as you would like. Please do not leave any blanks, even if you need to answer that you do not know. These comments will be much more valuable than just numerical scores and will help to make this a more effective exercise.
